    Notice of Privacy Practices Your Information. Your Rights. Our Responsibilities. ​ This Notice of Privacy Practices (“Notice”) describes how medical information about you may be used and disclosed and how you can get access to this information. PLEASE REVIEW THIS NOTICE CAREFULLY. ​ At ATLAS AIR AMBULANCE, LLC, we are committed to treating and using protected health information (“PHI”) about you responsibly. This Notice describes the PHI we collect, and how and when we use or disclose that information. It also describes your rights as they relate to your PHI. This Notice applies to all PHI, as defined by federal regulations. ​ Your Rights When it comes to your health information, you have the following rights: Get an electronic or paper copy of your medical record. You can request an electronic or paper copy of your medical record and other PHI we have about you. Ask us how to do this by contacting us via email at info@flyatlasair.com . We will provide a copy or a summary of your PHI, usually within thirty (30) days of your request. We may charge a reasonable, cost-based fee for providing copies of such records. Ask us to correct your medical record. You can ask us to correct health information about you that you think is incorrect or incomplete. Ask us how to do this. We may say “no” to your request, but we’ll tell you why in writing within 60 days. Request confidential communications. You can ask us to contact you in a specific way (for example, home or cell phone) or to send mail to a different address. We will say “yes” to all reasonable requests. Ask us to limit what we use or share. You can ask us not to use or share certain PHI for treatment, payment, or our operations. We are not required to agree to your request, and we may say “no” if it would affect your care. If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information for the purpose of payment or our operations with your health insurer. We will say “yes” unless a law requires us to share that information. Get a list of those with whom we have shared information. You can request a list (accounting) of the times we have shared your PHI for six (6) years prior to the date you ask, who we shared it with, and why we shared it. We will include all the disclosures except for those about treatment, payment, and health care operations, and certain other disclosures (such as any you asked us to make). We will provide one (1) accounting a year for free but will charge a reasonable, cost-based fee if you request additional accountings within twelve (12) months. Get a copy of this Notice. You can ask for a paper copy of this Notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly, upon your request. Choose someone to act for you. If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your PHI. We will make sure the person has this authority and can act for you before we take any action. File a complaint if you feel your privacy rights are violated. If you feel we have violated your privacy rights, you can contact us using the information provided under “Contact Information” below. You can also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting www.hhs.gov/ocr/privacy/hipaa/complaints/. We will not retaliate against you for filing a complaint. Your Choices For certain health information, you can tell us your choices about what we share. If you have a clear preference for how we share your PHI in the situations described below, please advise us and we will follow your instructions. You have both the right and choice to tell us to share PHI with your family, close friends, or others involved in your care. ​ If you are not able to tell us your preference, for example, if you are unconscious, we may proceed with sharing your PHI if we believe it is in your best interest. We may also share your PHI when needed to lessen a serious and imminent threat to your health or safety. ​ In the following cases, we never share your PHI unless you give us written permission: Marketing purposes; and Sale of your information. Our Uses and Disclosures How do we typically use or share your PHI? We typically use or share your PHI in the following ways: Treat you. We can use your PHI and share it with other professionals who are treating you. Example: A doctor treating you for an injury asks another doctor about your overall health condition. Run our organization. We can use and share your PHI to run our organization, improve your care, and contact you when necessary. Example: We use PHI about you to manage your treatment and services. Bill for your services. We can use and share your health information to bill and get payment from health plans or other entities.   • Example Transportation Timeline | Southeast Air Ambulance

    Typical Southeast Air Ambulance Flight Plan Here is how a typical Southeast Air Ambulance transport unfolds: Quote and In-Take Southeast Air Ambulance receives a quote request for medical transportation by web submission, email, or telephone and our team puts together a personal transport according to the patient's needs. A SEAA Medical staff member will obtain a thorough medical report, including all patient and additional passenger information. Our SEAA Medical Coordinator will evaluate the transport needs of the patient based on the information received. The information received will assist us in determining the most appropriate medical facility for treatment. Southeast Air Ambulance can arrange all hospital admissions for you. Arrangments and Confirmation The flight SEAA Medical Team is selected based on the patient’s condition and medical needs. The Medical Team may include a combination of: Physicians, Nurses, Respiratory Therapists, EMT, and/or Critical Care Paramedics. A departure time is set and the SEAA medical team is dispatched. Arrangements are made for ground ambulances on both sides of the air transport. The patient’s Health Insurance Company, if needed, is contacted with notification of the transfer. Updates on the transport itinerary are confirmed with the patient and family member(s). Southeast Air Ambulance pilots conduct a pre-flight check of the aircraft, obtain weather reports, file flight plans, determine fuel requirements, and arrange for landing Internationally. Departure and Flight The SEAA Medical Team reviews the patient’s medical report and loads medical equipment and supplies into the aircraft. The aircraft departs, en route to the patient. Pre-arranged ground transportation is waiting to receive and take the SEAA Medical Team from the airport to the patient’s bedside. At the discharging facility, the SEAA Medical Team assesses the patient’s current condition and receives an updated medical report. SEAA Medical Team prepares the patient for the transport and connects the patient to SEAA’s portable medical equipment: Oxygen, Ventilator, IV pumps, etc., as required. The patient is transferred to our portable stretcher and secured for the ground transport ride back to the airport. At the airport, the crew carefully transfers the patient into the aircraft. Once inside the aircraft the SEAA crew continues to monitor the patient while the aircraft departs. Upon arrival at the destination airport, the patient is safely transferred from the aircraft into the awaiting ground ambulance. Destination Arrival The ground ambulance then transports the patient, family member(s) and medical flight crew to the receiving hospital, clinic or private home. Once the patient arrives at the hospital, our SEAA Medical Team will give a medical report and hand over care to the receiving facility. The SEAA Medical Team then returns to the aircraft for their flight home.
